When you have an injury, all you want is relief from the pain. Well, pain relievers aren’t always in pill form. Sometimes, simple hot and cold can ease those aches.

Pulling a muscle or twisting an ankle can make anyone uncomfortable. The body’s response is a rush of cells that try to heal and protect the area, but those cells cause swelling, and the swelling causes pain.

If you don’t have an ice pack in the freezer, don’t worry. “I’ll tell a lot of patients to use a frozen bag of vegetables, peas or corn. You can also use a wet towel and put it in the freezer for a few minutes. You can make another simple ice pack by filling a Ziploc bag with water and adding about a half cup of rubbing alcohol. Hoff says freezing it will create a gel-like ice pack.

Freeze some water in a dixie cup, peel back the paper and massage for 3 to 5 minutes.

After the first 72 hours, use heat to relieve the pain. “Heat can be very soothing and help you get to sleep or get more comfortable,” Hoff says. Simply wet a towel under hot water, and put it in the microwave for a few seconds. Then apply to the painful spot.

A hot bath or shower could also relax stiff muscles, or try a heating pad on a low or medium setting, but no longer than 20 to 30 minutes. “You leave it on too long, and too much blood flow comes to the area, and you get increased swelling,” explains Hoff.

So ice, then heat, and before you get stiff, get back on your feet.

Hoff says if you’re pregnant, have high blood pressure, diabetes, or any vascular disease, check with your doctor before using hot and cold treatments.

Cambodian contradiction: Pain, beauty mingle in Southeast Asia

PHNOM PENH, Cambodia - Sweet shouts of “Hello!” greet our gaggle of volunteers from Massachusetts as the local kids, surprised to see Westerners so far off the tourist track, try out their limited English. It’s hard at every step to reconcile their openness and desire to learn with the knowledge that many are destined for a life of poverty, assuming they avoid malaria, AIDS and human traffickers.

“It is amazing how people seem so happy who have so little,” said Rachel Schatz, one of eight of us from Park Street Church in Boston who spent two weeks in this mystical but ravaged land in August. “Cambodians are all so generous.”

Our group spent two weeks in Cambodia, teaching English to adult staff members of World Relief, a church-based aid organization. Most of the time, I felt more like the student than the teacher.

The Cambodians I encountered embodied warmth and grace - impressive, considering their recent history. In the last 30 years the country has endured genocide, civil war, rampant government corruption, human trafficking and epidemics of poverty and disease.

Memories of the fantastic colors and textures of market fruits compete in my mind with grim images of piles of skulls at a monument to victims of the Khmer Rouge. Merchants make a thin living selling fine silks and crafts, while a band made up of land-mine survivors struggles for survival playing their music and selling their CDs.

Tourists flock to one of the world’s great architectural feats, Angkor Wat, built by the ancient Khmers around 1100, while Cambodians live in makeshift lean-tos along city streets or, in the country, in one-room huts on stilts.

A typical resident makes less money in six months than many Americans make in a week. Yet my students took us for soup and gave us gifts when we left that could have cost them a day’s wages.

Cambodia’s hope for change lies with nongovernmental organizations like World Relief. Its programs help mothers raise healthy children, teach kids about health and hygiene and give people a chance to start or expand small businesses (information: http://cambodia.wr.org/). There’s much reason to fear for the future of Cambodia. But seeing the excitement of the village children during lessons there, it’s possible to believe there’s reason to hope, too.

India’s Ranbaxy Lab says gets FDA approval for pain relief drugs

MUMBAI (Thomson Financial) - Indian pharma company Ranbaxy Laboratories Ltd said it secured Food and Drug Administration approval to manufacture and sell pain relief drugs Hydrocodone Bitartrate and Acetaminophen tablets US Pharmacopeia in the US.

Jim Meehan, vice president of sales and marketing, Ranbaxy Pharmaceuticals Inc said the new drugs — to be launched in November — will further expand the company’s portfolio.

Annual sales for Hydrocodone Bitartrate and Acetaminophen tablets in 2006 was 390.6 mln usd, Ranbaxy said in a statement to the Bombay Stock Exchange.

Anesiva Gets FDA OK for Zingo Pain Drug

SOUTH SAN FRANCISCO, Calif. -Biotechnology company Anesiva Inc. said Friday the Food and Drug Administration approved its Zingo powder injection pain relief system.

Zingo provides rapid relief for pain associated with venous access procedures, such as intravenous insertions or the drawing of blood for tests. It is aimed at use in children between 3 and 18 years old.

Rite Aid Aims at Pain Relief

Rite Aid turns its health spotlight on pain relief this summer in a campaign supporting private label products.

The chain offers $25 in coupons for free private-label pain relief products (including children’s and adult ibuprofen, aspirin and muscle rub) to shoppers who spend $20 on other pain relief SKUs. Participating national brands include Bayer Healthcare’s Aleve, McNeill’s Tylenol and Wyeth Consumer Healthcare’s Advil. Valid on purchases made between July 29 and Sept. 29, the offer is featured in the chain’s July 29 circular and August Single Check Rebates catalog.

The campaign also includes in-store distribution of a 12-page Pain Relief Guide and a Pain Prevention Booklet developed in conjunction with the American Pain Foundation, along with a one-sheet of coupons from such brands as Advil, Novartis’ Excedrin and Chattem Inc.’s Pamprin. The program is promoted on an endcap near the pharmacy merchandised with participating brands.

Breakthrough arthritis pain relief drug ruled too expensive for the NHS

Thousands of arthritis sufferers in the UK will be denied access to a new drug that could dramatically ease severe symptoms of the condition after it was deemed too expensive for the NHS.

Guidelines issued by health watchdog, the National Institute for Health and Clinical Excellence (NICE), does not recommend abatacept (Orencia) on the NHS which could help around 12,000 sufferers with severe arthritis in the UK.

NICE controls the flow of drugs on the NHS.

Around 400,000 people in the UK have rheumatoid arthritis, of which 10 per cent (40,000) have a severe form of the disease.

They can be offered a class of drugs called anti-TNFs but around 30 per cent of people will not derive any benefit from those. This group - around 12,000 patients - could then be eligible for a drug such as abatacept.

The National Rheumatoid Arthritis Society (NRAS) described the Nice decision as “devastating news”.

“Denying patients the option of abatacept leaves some of them with the unacceptable choices of being put back on to treatments they have already failed on, palliative care, or taking large doses of steroids which have extremely unacceptable side effects such as osteoporosis when given over the long term.

“We are talking about a relatively small number of patients who have a very aggressive and severe form of rheumatoid arthritis. They will have failed all other treatment options before they are recommended for abatacept.

“From a cost perspective, only those who attain sufficient benefit will remain on treatment. If there is no response then treatment can be stopped very quickly, so minimising cost.

“The guidance recommends denying our most severely ill rheumatoid arthritis sufferers one less vital step in their treatment pathway.

“We simply cannot accept that individuals should be denied the chance of at least regaining some quality of life and condemning them to a life of pain and disability, which could be equally as expensive to the NHS.”

Putting medical relief on track

Two days a week, through Boston’s Health Care for the Homeless Program, Carlo sees uninsured backstretch workers who would otherwise be without medical care.

With no bathroom or running water in the trailer, Carlo relies on hand sanitizer that she pumps from a bottle in between patients. Inside, eight chairs are crammed into the narrow space. A few feet from the chairs stands a free food pantry, stocked with jars of peanut butter, cans of noodle soup, and boxes of macaroni and cheese.

In addition to working at the Suffolk Downs clinic, Carlo, who lives in Newton, spends three days at Boston Medical Center. She treats homeless people in a clinic there and cares for HIV-positive patients.

“I feel really fortunate that I get paid to do work that’s very fulfilling,” said Carlo. “I feel like I’m really helping people.”

At the track, Carlo sees workers who spend their days training, feeding, exercising, and grooming the horses. She has been with the program — and many of the patients — for nine years. The majority are migrant workers who head to other tracks around the country when Suffolk Downs closes for the season in November. Some are from the United States; many are from Spanish-speaking countries such as Guatemala, Mexico, and El Salvador.

The nature of the clinic poses a difficult challenge for Carlo, as she will often get a patient’s medical condition under control (for example, diabetes or high blood pressure) and then have to hope they will continue taking their medications once they’ve moved out of her care.

Carlo works with registered nurse Trish Bowe at the clinic. In addition to addressing common clinic issues and procedures, such as sore throats, high blood pressure, flu shots, and Pap smears, Carlo sees industry-specific ailments such as jammed fingers from holding bridle reins and broken bones from a horse kick.

Lynette Bush of Haverhill said that having Carlo around is a big relief to her. “She can help whoever is sick or in pain; I know a lot of people are glad she’s here,” said Bush.

Over the years, Carlo has tended to Bush’s broken leg (the result of a horse kick), colds, and sore throats. Today Bush has come to see Carlo about a pain running down her left leg that she suspects is sciatica.

Carlo, a 49-year-old mother of three, has worked in mainstream facilities but says she prefers caring for the indigent population, a connection she first made in the early 1980s when doing her primary care residency at St. Vincent’s Hospital in New York City’s Greenwich Village.”It was both interesting and creepy,” said Carlo, recalling a visit she made to an elderly woman’s apartment that was filled with aging theater costumes. In some homes, she said, she did not want to put her purse down as “there was either so much junk around or there were roaches.

Paracetamol for pain relief after surgical removal of lower wisdom teeth

The commonly used and inexpensive analgesic acetaminophen (Tylenol or paracetamol) is an effective pain reliever after surgical removal of lower wisdom teeth, according to a new systematic review of 21 studies.

The most effective dose appears to be 1,000 milligrams taken at six-hour intervals, the review concludes.

“Acetaminophen has been around a long time. It has a good safety record and is widely available without prescription. Our findings suggest it is a good choice for dental pain,” said review co-author Kiaran Weil.

The review appears in the latest issue of The Cochrane Library , a publication of The Cochrane Collaboration, an international organization that evaluates medical research. Systematic reviews draw evidence-based conclusions about medical practice after considering both the content and quality of existing medical trials on a topic.

Researchers from the University of Manchester School of Dentistry in England and the University of Amsterdam in the Netherlands analyzed the studies.

The review examined data from 1,968 patients enrolled in studies that compared the effectiveness of acetaminophen versus placebo to decrease pain after surgical extraction of the third molars, or lower wisdom teeth. The review included patients who received local anesthesia, intravenous sedation or general anesthesia; however, it excluded patients who took pain relievers at the time of surgery.

All patients reported moderate to severe pain just after surgery. Researchers tallied the number of patients who reported 50 percent pain relief at four and six hours after surgery.

Patients who received any dose of acetaminophen after surgery were at least three times more likely to report 50 percent pain relief than patients who took a placebo. However, patients given the higher dose of 1,000 milligrams were four to five times more likely to report that their pain was cut in half. Study participants who took less than 1,000 milligrams were about twice as likely to report 50 percent pain relief.

However, researchers monitored patients taking acetaminophen, and Weil cautions that side effects are less likely to occur under controlled circumstances.

The maximum recommended dose of acetaminophen is 4,000 milligrams per day for adults.

Acetaminophen can cause liver toxicity when an individual exceeds the maximum daily dose. Heavy drinkers, malnourished patients and people with AIDS or anorexia nervosa also have an increased risk for liver toxicity.

While the review shows acetaminophen is effective for pain relief, oral surgeon Morton Rosenberg said many dentists prescribe analgesics plus narcotic pain relievers after surgical removal of wisdom teeth.

“This is a procedure where the dentist is working on bone and cutting oral tissue. These are some of the strongest stimuli for pain that we know of,” said Rosenberg, professor of oral and maxillofacial surgery at Tufts University School of Dental Medicine in Boston.

In his practice, Rosenberg treats pain after wisdom tooth removal on a case-by-case basis.

“It varies so much from patient to patient that it is hard to generalize. I do often give a prescription for something like Tylenol with codeine and advise the patient to fill the prescription if needed,” he said. Many patients expect to receive something stronger than an over-the-counter medication, he added.

Future reviews of studies will compare acetaminophen directly to other pain relievers such as ibuprofen.

After one year it was found that those being treated using acupuncture not only had reduced amounts of pain but they found themselves also worrying less about the pain as well. Whilst after carrying out the treatment for 2 years the patients being treated using acupuncture were reporting that they were in most cases pain free from the second year and were now beginning to use less medication.

When it comes to using acupuncture as a natural back pain relief treatment, a person can generally expect to pay between $60 and $120 for the treatment but it is tax deductible as it is considered to be a medical expense.

Also if you are considering using this method to treat your back pain then it may be wise to check your medical insurance plan as the provider may well cover the costs of such treatment within theirs. Plus, you need to be aware that initially you will need to see the acupuncturist between 1 and 3 times a week for several weeks in the beginning before the effects can be felt.
